Daniel Reinhold 953aa0fc95 fleshed out some tty stuff... 
isatty() is fixed - altho this is basically academic since we don't have any
drivers for terminal devices written yet (but at least the code is ready when we do)

also, ctermid() is implemented - which is of no big consequence

Daniel Reinhold 70dee992a3 style changes -- purely cosmetic 
normally, I wouldn't checkin a file with cosmetic changes only
however, Marcus just made a change on this file earlier today,
so the file would have to be rebuilt anyway -- might as well
make it pretty too!

Daniel Reinhold ca60e89ab7 replaced syscall with find_thread(NULL) to obtain current thread id 
currently, find_thread(NULL) also uses a syscall, but in the future,
this will be implemented as an efficient inline lookup in calling
thread's local storage

Daniel Reinhold 14bc22fb88 some cleanup and reorganization: 
- the inline functions I introduced earlier have been removed
- the notes about non-Posix extensions to the sigaction struct have been
  updated, enlarged, and moved out of the way (near the bottom of the file)
- all the function prototypes have been collected and placed together
- a few more items have been commented
- in general, the organization of the whole file should be more tidy now (I hope)

Daniel Reinhold c69a62db20 several fixups: 
a new typedef for signal handlers - sig_func_t
(the old one is kept around for backwards compatibility)

a new macro MAX_SIGNO to get rid of magic 32's all about

two more functions (sigemptyset and sigfillset) are now
defined inline as well
