* ServerApp was accessing ServerWindow::Window() (while having the app window
lock held), but in fact, there was no guarantee it already existed, or was
added to the Desktop.
* Therefore, the Window() semantics have changed to only return a window in
case the window exists *and* has been added to the desktop (the latter
constraint might be lifted again, though). Therefore, it doesn't work
for offscreen windows, and should not be used within ServerWindow code
anymore.
* This fixes bug #686 and maybe others as well.

Renamed lots of things, like ScreenSaverThread to ScreenSaverRunner,
refactored code, etc. Much cleaner interfaces and code.
* Fixed a couple of bugs and in the add-on handling, especially some settings
related bugs (ie. testing a screen saver will now use its latest settings,
etc.).
* Correctly implemented DPMS support in ScreenSaverPrefs and the ScreenSaver
preferences application - screen_blanker still ignores them, though.
* It's not yet font sensitive either.
* Changed the input_server add-on to not switch to the screen blanker immediately
when it's in the "blank corner" - doesn't seem to work yet, though (only tested
under Qemu).
* Correctly implemented the "preview" function (before, a screen saver would
never know it rendered a preview).
* Evaluates the return value of BScreenSaver::StartSaver().
* The screen saver thread is no longer killed without notice - it's now
always shut down properly.
* Made the code more robust against failure.
* Introduced some new bugs as well (the screen saver list view doesn't jump to
the selection anymore, for some reason), those will be fixed later (as the
remaining issues).

* No longer opens an empty document for every non existing path - it now only
opens a new document if there is none yet.
* No longer open an empty document for non existing files - instead, it just
passes the ref to StyledEditApp::OpenDocument(). StyledEditWindow::_LoadFile()
now treats those files gently as well, so that you can create new documents
via StyledEdit as on BeOS without getting annoyed too much.

mapping, but it connected the vm_cache objects, so if it failed later, and
thus called vm_cache_release_ref() the object could have been freed accidently.
Most uses of map_backing_store() explicetly acquired a cache_ref *after* the
call was successful, but _vm_map_file() did not do this.
_vm_map_file() might still not work correctly, though, need to have a closer
look at it.

* We now write the META:TYPE attribute that contains the type with
preserved case (as in BeOS). We even currently check this whenever
a type is opened, but that can be removed later, again.
* BMimeType::GetSupportingApps() was broken for applications that
only support the super type.
* Also BMimeType::GetSupportingApps() preserves the case of the
signatures.
* Why on earth would we want to preserve the case of case insensitive
MIME types? Simple, this let's you query for those apps using case
sensitive queries, as Tracker uses for its "open with" functionality.
Strangely enough, that one still doesn't work, even though the query
predicate is now correct.

let them eat death stack entries: after setting the next thread state to
THREAD_STATE_FREE_ON_RESCHED, interrupts were enabled again, which could
cause the thread to be rescheduled before having called put_death_stack().
This fixes bug #434.
* Note that the above change pretty much reverts revision 7865 that was supposed
to fix interrupt problem on thread exit (patch by Jack Burton almost 2 years
ago, that's how long this problem existed!).
* Made get_death_stack() and put_death_stack() symmetrical in that they don't
change interrupts. Also pulled out rescheduling from put_death_stack[_and_reschedule]()
and put it back into thread_exit2().
