Michael Lotz b204648b75 Don't define UNIX when building zip as a build tool under a BeOS compatible
host. This prevents the BeOS and Unix specific zipup.h from clashing and allows
hybrid GCC2/GCC4 images to be built under BeOS.

Michael Lotz 9a18e1d269 * Do not delete the device handed over to the devfs when deleting the vnode
as it seems devfs should not take over ownership. The device is deleted
  in republish_driver for legacy drivers before the call to devfs_unpublish_device
  that in turn deletes the vnode. This looks generally suspicious to me as
  the device pointer the devfs vnode may still use gets invalid for a certain
  amount of time (until the node is unpublished). So maybe the devfs should
  take ownership of the device afterall and it shouldn't be deleted by the
  device_manager/legacy driver functions. Axel please review.
* Do not try to delete the IOScheduler for now as creating one is disabled
  currently too.

This fixes the crash on rescans of legacy drivers as seen when unplugging
USB devices.

Michael Lotz c7d757a074 * For multi function devices it is not so unlikely to try to do multiple
concurent SendRequest() calls. Therefore it's probably a good idea to
  protect that, now that the request data is not dynamically allocated anymore.
* Fix warning about not returning a result in the compatibility version of
  init_mutex_etc() (which wouldn't make a difference as the Haiku version
  does not return anything).

Michael Lotz 9e48bf8e42 Rewriting the usb_hid driver. It has now gotten a driver part and a device part
structured into the HIDDevice base class and KeyboardDevice and MouseDevice
subclasses. This can be extended easily to support more device types like game
controllers, joysticks and other HID devices. The parsing code remains untouched
while the interpretation code has been integrated into the device classes.
The driver should work much the same way as before including the boot protocol
only keyboard limitation. On the other hand composite devices that combine
multiple devices should now work, as long as they expose those devices as two
seperate interfaces.

Michael Lotz c2f077d03e * Clear the configurations list so it can safely be handled in case the device
fails to init completely.
* Fix wrong usage of realloc and check the allocations.
* Check a few more allocations, should be all now.
* Fix a small memory leaks in error case.
* Do a few more checks when unconfiguring and deleting a device so an incompletely
  initilized device is deleted correctly and does not crash or leak.

Michael Lotz b30304acc8 * Add a DumpFontList test that dumps all families and styles with most BFont
infos (should all be defaults expect for a few).

This shows that with our default installation there are quite a few "condensed"
styles and also an "extra light" variation. The condensed ones should probably
get their own face flag. The "extra light" and other unhandled styles should
probably not be classified as a B_REGULAR_FACE as this could lead applications
to use them in a wrong way yielding unexpected results (i.e. firefox that
picks up the "extra light" style for it's interface font).

Michael Lotz c1990557b8 * Use a lazily initialized notification sem for all SendRequest() calls.
* Also use member variables to transport the status information from the
  callback instead of allocating a dedicated structure each time.
* Remove the now unused transfer_result_data struct.

This should bring down some overhead otherwise involved with sending requests
over control pipes. As this is heavily used in updating for example the hub
status (also on roothubs), this change should make the polling loop a lot
cheaper. Note that this makes SendRequest() non-thread safe, but as it is a
synchronous function (as opposed to QueueRequest) and since the underlying
architecture is not thread safe either this shouldn't be a problem (drivers
doing a SendRequest from two different threads would have failed previously
anyway). A benaphore or mutex could be employed to fix that if the need really
arises.

Michael Lotz 307fe93f85 * Disable the root hub change interrupt when it gets triggered. As the change
status stays on until the port is reset, this causes the interrupt to be
  constantly retriggered on some controllers.
* Added TODO that we need to reenable it once we want to take advantage of
  status change notifications instead of polling. It should be reenabled when
  clearing roothub status changes in ClearPortFeature().

Michael Lotz 5b03912b5f * Correct removal of failed transfers by applying the address mask. As the head
pointer also contains the toggle carry and the halt bit (which was obviously
  always set as the endpoint had an error) the address comparison failed and
  the head was dead in the water with all further transfers timing out until
  a subsequet cancel cleaned the mess up again.
* Add the OHCI host controller driver to the image as it should now be fully
  functional except for isochronous transfers.

Michael Lotz 519fbf77d3 * Make the maximum port count a define and set it to 16. OHCI officially
supports up to 15 ports and QEMUs OHCI emulation uses a port count of 10
  for example.
* Update the hub code to remove the hardcoded 8 port limits.
* Some other code to enumerate the USB structure I had laying around
  (available internally only, might be added as a public API some time).

Michael Lotz a5a728361c * Build the interrupt tree the same (imperfect) way as in EHCI to conserve
on resources but still satisfy the scheduling policy. This works well, but
  effectifly limits the bandwidth available as the interrupts aren't spread
  across different frames.
* Implement finding the interrupt endpoint head for a certain interval.
* Deactivate endpoints that are going away until removing them is implemented.
* Allow short packets for all data phases.
* Disable tracing by default.

There you go, interrupt transfers should work as well. Should enable mice,
keyboards, hubs and other devices that use interrupt pipes. Note that it's
still easily possible that a single failed request will hang an endpoint and
prevent a device from working.

Michael Lotz 016ce5dd54 * Handle the data == first and last == first case as this can occure for
single td bulk transfers.
* Implement SubmitTransfer() for bulk transfers (interrupts will use the exact
  same code path, but as their endpoints aren't yet setup, interrupt transfers
  won't work).
* Handle the cancel case when finishing transfers. The descriptors of the
  canceled transfers cannot be accessed by the controller so they can be freed.

Bulk transfers should work now, so devices only using control and bulk transfers
should too (anything using usb_disk for example). Note though that a transfer
error will cause the whole thing to fail miserably as the error case is not
yet handled correctly (failed descriptors aren't removed from the endpoint).
Therefore I suggest not testing with the memory stick with that important
presentation you haven't stored anywhere else...

Michael Lotz 408a8ff36a * Completely rework how transfer finishing is implemented. It is now using
pretty much the same strategy as UHCI. This does not leverage the done queue
  the hardware provides us, but as it saves a lot of other overhead and allows
  the structures to be smaller I think it is overall worth it.
* Removed now unnecessary stuff from the transfer descriptor struct.
* Directly acknowledge the done head interrupt as we do not use it either.
* Activate control endpoints by removing the skip bit when a transfer is scheduled.
* Correct the way the first descriptor is determined (the current tail becomes
  the first descriptor while the current first descriptor becomes the tail).
* Remove the head_logical_descriptor field from the endpoint structure, as this
  would simply get out of sync with the first td with no real way of updating.
* Stub out RemoveTransferFromEndpoint() as this needs reworking.
* Correct CreateDescriptorChain() signature and implement the method.
* Add ReadDescriptorChain() and ReadActualLength() to process finished transfers.
* Prepare for actual data transfers by renaming/regrouping transfer types.
* Fully clear out the new tail when switching (for the sake of it)
* Add some helpers and definitions to the hardware header.
* Add debugging facilities.

With all this transfers can now actually be scheduled and they are processed
when done. This brings control transfers to a usable state so setting the
device address and reading out/setting the configuration works. That means you
can now view your device using usb_dev_info, but since any other transfer type
besides control transfers isn't implemented yet the device will most probably
not yet do anything useful.

Michael Lotz a9d7e87d40 * Implement "tail switching" so that a new transfer descriptor chain can be
appended to an endpoint descriptor without locking/disabling the endpoint.
* Correct the direction of the data phase of a control transfer.

Control requests without data phase (set address for example) might actually
work now, but this is still untested.

Michael Lotz 20bbb1bf28 * Make the operational register memory a uint8 * instead of a uint32 * so the
driver has at least a chance of working (it previously always used wrong
  offsets for register access).

Michael Lotz f97c4f2fdc * Implemented transparent device replugs. If the same device (by MAC address)
is plugged in after having been unplugged the device will now reuse a still
  existing ECMDevice object. This allows for the link simply going down when a
  device is unplugged and going up again when the device is replugged. The
  nice thing is that due to the way our usb drivers work it doesn't matter
  where you replug the device, so you can switch it from one port to another
  or even from a highspeed to a fullspeed bus transparently.
* Fix a race condition between the notify hook and the device removed hook.
* Added some comments and extended some debug output to be more useful.

Michael Lotz 6ba2a6764d * Reset the connection state and speed when the underlaying device is removed.
* Notify the stack of this by releasing the link state change sem (if present).
* Also check for a removed device in the notify callback to prevent accessing
  a device that is going down.

Removing the device under Haiku with an active link will now report the loss of
connection. Still left to implement is to reuse a still existing device if the
same device (based on its MAC) is replugged.

Michael Lotz b995c2834c * Added support for device state change notifications and implemented the
ETHER_GET_LINK_STATE and ETHER_SET_LINK_STATE_SEM to provide this
  information and notification.
* Don't try to clear an endpoint halt in case of a B_CANCELED status in the
  callbacks as this is triggered by explicitly canceling transfers when the
  device is closed/removed.
* Renamed the semaphore members to *Sem to distinguish them better.
* Some minor other cleanup and some added comments.

Michael Lotz 745c6cce89 Adding USB ECM (Ethernet Control Model) driver. This driver should support
devices of the CDC class (2 - communication) with ECM subclass (6) interfaces.
This type of device can be for example a USB to ethernet adapter or current
UMTS cell phones that support the Wireless Mobile Communications Devices (WMC)
standard.
Note that there is also another, similar, thing called EEM (Ethernet Emulation
Model) which we want to support too and is why I called this driver "usb_ecm"
instead of just "usb_network".
This driver was written in less than half a day and while it works nicely with
my Sony Ericsson K850i (comitting over this very device/driver), it does not
yet contain features like link state reporting and multicast and may obviously
contain a few bugs.

Michael Lotz bb90c2a603 * Clear the endpoints of an alternate interface that is going away before
updating the active pointer as otherwise the endpoints of the new alternate
  would have been cleared instead of the old one (leaking endpoints).
* Only clear and re-initialize the endpoints of the interface we are setting
  an alterntate. Otherwise we tear down all the device endpoints which would
  result in a bad situation for composite devices where multiple drivers may
  use different functions (through different interfaces) of a device side by
  side.
* Minor cleanup.

Michael Lotz de39891a3c Add the "-fno-tree-vrp" option when building with GCC4. It seems that the
value range propagation optimization results in broken code with the GCC4
version we are currently using. Added TODO to retest/remove this flag when we
update our GCC4 compiler.

Michael Lotz 4b80900d33 Remove the at least questionable b_sqrt() function and replace it with the
normal sqrt(). The stated goal of the function was to avoid a dependency to
the math library, which in our case does not apply anyway. This removes one
case of broken strict aliasing rules...

Michael Lotz 0eaadd30b2 * Applying patch by Dustin Howett that updates the iso9660 filesystem to the
new filesystem interface.

Michael Lotz 72010d4247 * Ported the FAT filesystem to the recent filesystem interface changes which
was, thanks to the detailed instructions from Ingo, quite doable.
* Ported the (deprecated and now publically removed) notify_listener() calls
  to the respective new notify_* calls.
* Fixed debug build.
* Added the FAT filesystem back to the image.

I only tested the filesystem briefly and everything seemed to work as expected
(mounted the FAT part of my bootable Haiku memory stick, opened a few files
and copied something to it). At least it should not be any more broken than
before the interface changes ;-).

Michael Lotz f3283bfa83 * Increase the amount of tries in the EHCI BIOS handover so it lasts for up to
1 second instead of half a second. Might help with slow BIOSes.
* Disable EHCI interrupts if the BIOS handover fails so that we do not flood
  the system with interrupts if the BIOS later on decides to still comply with
  our handover request.
* Add an unconditional success message to EHCI and UHCI to better see if
  controllers are present/initialized correctly at all.

Michael Lotz 33574eaf6e Exclude the frame list rollover interrupt from the interrupts we are interested
in. We do not enable this interrupt and if we still "handle" it in our
interrupt handler we might prevent the interrupt from reaching the real intended
handler in a shared interrupt setup. Not sure if this could have happend at
all, but this is more correct in any case.

Michael Lotz c8661c3885 Make a local copy of the passed in isochronous packet descriptors before
handing them over to the USB stack. Otherwise the finisher (kernel) thread
would have no way to fill those packets when they came from a user space
address range. Completely untested though.

Michael Lotz 349a9cfc0f Use a list of host controller modules instead of using open_module_list() and
read_next_module_name(). This is ugly and less flexible but it allows us to
explicitly force an initialization order. As detailed by the added comment
there seem to be controllers/BIOSes that require the EHCI controller to be
initialized after the companion host controller or else they refuse to publish
any high speed devices. This was the case for bug #1236 and my laptop when
legacy emulation is enabled and any device was plugged in at boot. This change
fixed the problem for me and hopefully closes bug #1236 too.

Michael Lotz e959baed58 * Set the interrupt threshold of the controller when doing the rest of the
initialization as suggested by the EHCI spec and do not touch it again
  once the controller is running. Doing so would result in undefined behaviour
  according to EHCI specs.
* Set the port routing policy after the controller has started and not before
  as suggested by the EHCI specs.
* Explicitly set the segment register after resetting the host controller and
  not before, as this likely has no real effect ;-)

Might help with some legacy support or UHCI/OHCI to EHCI handover problems.

Michael Lotz dbcfdd5ca7 Correct the way serial devices are deleted. They are either deleted on removal
when they are not open or they are deleted on free when they are already
removed. This should fix the sudden crashes when you unplugged a device that
might not have been fully closed yet.
Also handle the case of removal correctly and don't use the usb_device anymore
after releasing it by returning from the device removed hook. Calls to the
device just return B_DEV_NOT_READY in that case.

Michael Lotz 017d539c2a A few more things to get the USB stack working under BeOS again. We now have
KDEBUG on by default which causes benaphores to always use their semaphore.
Therefore when initing the benaphore with the inline compatibility function
and then using it using the lock.h provided inline would always cause a
deadlock and a hanging system under BeOS. Also we now (re-)define the
B_KERNEL_{READ|WRITE}_AREA defines for non Haiku targets to 0 apparently which
would probably also cause the stack to misbehave. Therefore they are now
just redefined (again) to plain B_{READ|WRITE}_AREA in BeOSCompatibility.h.
Change the inclusion order in some places so things work as expected.

Michael Lotz d419b482ad Integrate the USBKit into libdevice and make it public. I left the USBKit.h
name, as it in fact is like a kit in the kit. It combines the different BUSB*
classes as compared to SerialPort.h where there really is a 1:1 relationship
between the header and the (one) class. If someone has a better fitting name
please shout.
Separated the usb_raw.h into usb_raw.h which defines the protocol and
usb_raw_private.h which holds the internal device structure for usb_raw. This
reduces the header dependencies.
Hopefully cought all occurencies of USBKit.a usage (in usb_dev_info and
the usb_webcam media add-on) but might have missed something that is not in
the image.

Michael Lotz 3568352d8d When a partition was unpublished from devfs the devfs_delete_vnode() would
also free the device info which it must not do, as this info is in fact just
a pointer to the device info of the raw node. Removing a raw device that had
partitions published therefore always lead into KDL when closing the raw
device after unpublising the partition, as the close_hook pointer which sits
in the device info now was 0xdeadbeef. This should make for example unplugging
USB devices work as expected.

Michael Lotz efb965d559 Introduce vfs_stat_entry_ref() that allows to stat a vnode based on a dev_t

Michael Lotz f1a9c3c214 Implement a downcounter for the synchronize cache command. There are devices
that do not support it (probably because they do not have a cache at all like
in flash media) and this should at least avoid spamming the syslog when this is
the case. It will try 5 times and then disable syncing for that device. Untested
as of yet though.

Michael Lotz a380bc2513 * Add support for devices with multiple LUNs
* A device now has one or more LUNs which are used for LUN specific functions
* Adapted these LUN specific functions to the new design and also change the

* Handle "no media" cases more correctly
* Output some interesting device infos also with tracing disabled
* Add some more helpful debug output here and there

Devices that provide multiple logical units (like those n in 1 card readers)
should now be usable with usb_disk too. Also we don't fail when a device is
attached which currently does not contain a media. Instead handle media changes
correctly so inserting a media works as expected. Note that hot-unplugging and
media removal still does not quite work right at the devfs or disk device
manager level and easily crashes the system.

Michael Lotz 06da81f012 Implement node monitoring in the kernel disk device manager.
* Added {Create|Delete}Device() analogous to {Create|Delete}FileDevice
* Added a small DeviceWatcher class that reacts to entry creation/removal
* Implemented a way to start/stop node monitoring
* Start watching for devices after the boot volume has been mounted and the
  the second initial scan was run

The disk device manager now creates and scans a device when a "raw" node is
published and deletes the device on removal. This makes hot-plugging of disk
devices (for example memory sticks using usb_disk) work. Their partitions will
be scanned and published so they can be mounted. Somehow the removal of the
partitions does not yet work however, any insights are welcome.

Michael Lotz 2509807a53 Change publish_node() of the devfs to not insert the node into the directory.
Instead return the directory vnode, so the calling function can insert it after
all vnode initialization is done. Otherwise entry created notifications are
sent upon inserting the node into the directory for vnodes that are not yet
fully initialized (i.e. they have no type or the device hooks are not yet set).
That would cause for example ioctls to fail when someone would react to a
notification of a newly created device.

Michael Lotz a93bd0c452 Cutting down on unnecessary per-controller delays when starting up the USB
stack. The root hubs do not really need a powerup delay and as they are not
used until all initialization is done, this gives plenty of time to stabilize
the power. Therefore remove the powerup delay for root hubs and reduce the
stabilization time to zero. Also remove the define for the first explore delay
as it is not used.

Michael Lotz 01533b17a2 Introduce a simple usb_disk driver that supports USB mass storage devices of
the bulk-only class using transparent SCSI commands (i.e. most of the current
external harddrives and flash media). It emulates the few SCSI commands needed
to get this sort of devices working and does not interface with the SCSI
subsystem. This makes it far easier to get working and also far better fits
how the USB stack works (as drivers can be dynamically rescanned when device
changes occur). This will allow for easy dynamic un- and replugging at runtime.
Note that while the device is currently published when you plug something in at
runtime, the partitions are not scanned and published automatically. It works
however if you have the device plugged in at boot.
I haven't added this driver to the image yet, but if properly installed and
with boot module links to the driver and USB stack, it allows to boot Haiku
from USB media like memory sticks or external harddrives.

Michael Lotz 550f245a9d This was probably changed by accident in r22886. It caused all read/write
pages calls of the fat filesystem to return an error instead of the effective
result. Please shout if this was done on purpose. I tested a bit with a fat
volume and where it failed previously when opening files it now works as
expected.

Michael Lotz f4af1fba51 Use the empty_index field as the allocation_id for large allocations and just
use the index of the first page of the allocation as an id. This removes the
need for separate id generation. This also fixes the possible problem of
multiple large allocations getting the same allocation_id (due to the limited
range of possible ids), which in the worst case (i.e. for adjacent allocations)
could cause pages to be freed that were still in use.

Michael Lotz a433b9febf Rewrite and activate message passing by area. Passing by area works now with
the new sematic of transfer_area so a message area is transfered into the right
teams' address space and it does not need to be cloned there anymore. Passing
by area is only used for messages bigger than a certain size (currently
hardcoded to 40KB) which should be somehow bound to the max port message size.
This makes passing large messages (i.e. > the port limit) possible, so for
example copy&paste of long text, image data, etc. should now work.
Got rid of the fClonedArea member as it is not necessary with the new design,
renamed shared_area to message_area in the private message_header, avoid
an unnecessary allocation of the header for the copy constructors, check
allocations in a few more places and some minor cleanup.

Michael Lotz d1189f0b05 Naive implementation of transfer_area(). It follows the suggested sematics
of the resolved ToDo, but could probably be made more efficient. Instead of
transfering the area, the area is cloned into the target teams' address space
and the original is deleted. This generates a new area_id for the transfered
area (as suggested by the ToDo). Updated syscall prototypes according to the
status_t to area_id return type change.

Michael Lotz 87df690150 * Replaced the UTF8ToCharCode function with one that handles invalid chars
correctly (or at all...)
* Use the common 0xfffd character as a substitute for invalid characters
* Corrected comment

This function is used by the app_server to convert the UTF8 strings to char
codes to feed FreeType. Using a non space substitute character at least for
now, as it makes it more obvious where invalid characters are present.
I tested this change with some UTF8 test files and it seems to work well.

Michael Lotz f566ae596d The fNeedToUpdate member was not initilized which caused the initial creation
of the image to be skipped. Magnify therefore showed a white frame as long as
the area around the cursor would not change. Fixes bug #598 (again). I wonder
why this didn't show up earlier though.

Michael Lotz 6dffd08db7 Hopefully this is a good compromise. If more than 99% of the interrupts of a
vector are unhandled, it is disabled if it is not a shared interrupt vector
(as before). Otherwise a message is printed to indicate the condition. I
reduced the amount of interrupts to 10000, as with 100000 it would take over
half an hour on my machine to trigger. Feel free to adjust.

Michael Lotz 94fec641a7 Implemented a small kernel debugger add-on that triggers a redraw of the
entire screen when exiting the kernel debugger. It sets up a thread that sends
a message to the (currently hardcoded) desktop message looper. The desktop then
does mark the whole screen dirty which causes a full redraw.
Since interrupts need to be enabled I went with an asynchronous thread and
releasing a request sem in the add-ons' exit hook.
Added the add-on to the image as it shouldn't hurt to have it for now.

Michael Lotz cf46897b4c Use a dedicated heap to allocate everything that is needed during heap growth.
This eliminates the edge case where the grow thread would not be able to create
a new area because no memory could be allocated for the allocation of the area.
As this case cannot happen anymore, it is also not possible to deadlock in
memalign. Therefore the timeout (which would only have prevented the deadlock
but wouldn't have solved the edge case anyway) has been removed too.
Add options to dump the dedicated grow heap and to only print the current heap
count to the "heap" debugger command.

Michael Lotz aabef40563 * Add a "stats" argument to the kernel heap leak checker to only print the
total count of allocations and bytes.
* Also add a few more bin sizes (for 8, 24 and 48 bytes) turns out especially
  allocations of 20-24 bytes are pretty common. And as it only wastes a few
  bytes per page this doesn't hurt at all.
